Impaired na excretion retention blood volume expansion increased cardiac output: raised cellular sodium prevents calcium transport out of smooth muscle cell raised pvr. Sympathetic activity: sympathetic activity influences co and pvr. Increased activity elevates pvr & causes volume expansion: renin plasma levels are raised in only a small number, many respond to ace inhibitors. Management: attend to risk factors, reduce peripheral vascular resistance.
