Nosocomial infection: an infection that is acquired directly from healthcare. Four pillars of safety strategy: a systems approach: policies, requirements (i. e. police clearances for workers, culture counts: a culture of safety, patients as true partners, bias towards actions, being proactive. What things affect safety: developmental level, age, lifestyle, environment, emotional and social context, current health status and perception. Increasingly complex cases and thus increasingly complex care-people are living longer with more chronic diseases: budget constraints-lack resources needed, antimicrobial resistance- bugs are immune/resistant to antibiotics. A pressure injury is a localised injury to the skin and/or underlying tissue. They are usually located over a bony prominence and occur because of pressure. Immobility: external pressure/ friction/shearing forces, poor nutrition and hydration because it makes the patient more vulnerable, damp/wet skin-this makes the skin more likely break down and be prone to pressure injuries. Increased age: thinner skin, easily torn, less fat, more bony prominence and the skin doesn"t repair as quickly.
