Testes develop in the abdominal cavity during the fetal life. Descend into the scrotum 7-9th month of gestation. M/c in premature & low birth weight infants. Spontaneous descent rarely occurs after 6 month of age. Gonadal arteriography & venography: look for blood supply of gonadal organs. Orchiopexy (surgery 6 months 2 years) Accumulation of excessive fluid between the layers of tunica vaginalis. When symptomatic: pain, heaviness in scrotum testes is pushed to one side. Dilation of veins in the pamoiniform plexus of veins (a network of veins draining the testes, eventually drains into ivc) Because of the inferior vena cave: on the left side, blood drains into renal vein, but on right side, it drains into the ivc 90 degree. Upright position force of gravity (leads to venous dilation: due to gravity, veins can"t bring the blood back and leads to venous dilation) Embolization just close it up, so the blood can move through.
