Thalidomide is a drug given to pregnant women to helping with sleep but caused birth defect. The differences detected more likely fur to treatment. Randomized assignments but little control of challenges or environment. If done well, it will provide stronger evidence for a treatment or preventive action than observational studies, plausibility arguments, case studies or anecdotes. One group gets treatment the other gets nothing. One group gets new, the other gets old. Some may need lower dose to see dose-response studies. Established treatments that was determined a long time ago. Tells the subjects or caregiver (for animals) what should and could happen. Making sure that the study is done in multiple places / conditions to increase the coverage and be able to extrapolate. The group still gets active component but not the treatment of interests. Placebo should be exactly the same to the treatment but without the active ingredient. To reduce any subjective bias from the subject or caregiver.
