The joint commission found 65% of sentinel events were the result of communication problems. These problems are not solved easily as they often are embedded in an organization"s processes, spanning social, cognitive, and technological factors. Nurses play a critical part in improving communication. Nursing leaders look to nurses to facilitate process improvement and technology integration. Can make communication more consistent and predictable. Builds social capital and legitimacy for nurses in their interactions with physicians. Occur at least twice a day for every patient, but the process rarely is taught in basic nursing education. A form of handoff, typically are unstructured and inconsistent. 4 types of shift report are typically utilized: verbal reports, tape recordings, phone calls, and written reports. Information retention by staff was better when written and verbal reports were combined.
