Medical dominance was de ned as the exercise of inconsistent power and influence in the process of discharge decision-making by doctors disproportionate, that is, to their position as only one of several professions represented within the teams. The theory of medical dominance, or professional dominance, has been central to many sociological studies of health care delivery. Medical dominance is viewed as social, cultural and professional autonomy independence and control over the content and context of medical care. The theory of medical dominance has been applied in many countries to explain the control of institutional arrangements of health care delivery, most notably in the usa, canada, and australia. The concept of medical dominance, as used in sociological writings, is similar to the concept of ceremonial (or institutional) dominance used by institutional economists. Institutional change is the phase of institutional adjustment in which instrumentally warranted standards of judgment displace ceremonially warranted standards of judgment in the correlation of behavior .
