Mandated by congress to develop guidelines for treatment. When the agency recommended less back surgery, political pressure from back surgeons almost forced elimination of agency. Now agency has recovered and budget has grown. System is decentralized, fragmented, poor communication - focus on improving it. Drug naming, packaging, labeling should be changed to minimize confusion. Managed care plans are easier to evaluate than other plans. Can evaluate quality of care for some procedures. Medical care is rationed by ability to pay. Racial inequities in care, even when ability to pay is same. Blacks and hispanics receive poorer treatment for many conditions. Less likely for coronary bypass surgery to be recommended. Childhood asthma- less likely to be given prescriptions for effective medications. Less likely to be recommended for mammograms. Less likely to receive antiretroviral therapy for hiv. Relative importance of medical care for public health. International comparisons show health is proportional to wealth (national gdp)
