Activity completely safely, without modification, assistive devices or aids, and within reasonable time. One or more of the following may be true: Setup occurs and pt is allowed to complete task on their own without someone in the room. Pt requires no more help than standby, cueing or coaxing without physical contact. Someone is needed to set up items, apply orthoses. Requires supervision and/or verbal cues to complete task. May not always be done safely or correctly. Pt requires occasional contact to maintain balance or dynamic stability. Pt expends 75% or more of the effort to complete task. Pt expends at least 50% (not more than 75%) of the effort to complete task. Pt expends at least 25% (no more than 50%) of the effort to complete task. Dependent/total assistance pt expends less than 25% of effort to complete task. Unable to initiate and/or perform any part of the activity on their own.
