Health history: past health, accidents, trauma, surgeries on joints or bone, why was surgery indicated, rehab post surgery, functioning or back to restorative function, family health, back problems, scoliosis, types of arthritis, rheumatoid, osteoarthritis, gout. Problem based history: movement, how long, are joints swollen/hot/stiff, muscle weakness, knees/ankles give way, adl. Shoulders at same level: muscle spasms with movement, rom of thoracic and lumbar spine, turn/twist side to side, touch their toes, hands on hips and bend to test lateral flexion. Inspect shoulder girdle: symmetry of location/shape, bony prominences, palpate shoulders for tenderness/masses, palpate elbows for tenderness, swelling and rom, dominant side may be larger , check strength/rom, straight extension, back extension, abduction, adduction, shoulder rotation. Inspect knees for symmetry & do they align: line up with tibia, do ankles have lateral of medial deviation. Joint itself should be smooth & non tender: note any edema/warmth/rom.
