Informed Consent
o Adequate disclosure
What is goig to happe what ould happe what will happe if we do’t do it
We are not allowed to talk about the specifics of the surgery. We are allowed to talk
about the general things that will happen for the surgery. We can't speak for the
surgeon. We only can answer questions that are in the scope of nursing practice.
We then need to refer back to the surgeon
o Clear understanding and comprehension
o Give Consent voluntarily
Families are really good at cohearsion and guilt tripping.
Physicians need to go in and do a PARQ note
o PARQ note
Record for
What is happening in the procedure
What are the alternatives of other surgeries or no doing the procedure
The risk for the surgery
Questions: have they answered all the patients questions?
We need to make sure that all the questions are answered before we get informed
oset. We ay eed to all the surgeo ak. A sigature does’t ea ifored
consent.
We cant obtain actual informed consent legally.
PARQ note happens before we ask for final consent and signature
Preparation of patient
o Surgical gown-check about underwear
Depends on where the surgery is being conducted.
o Nail polish
Dark and metallic nail polish is a no go. Most of the time we need to take off one nail.
Do’t eed to take off aryli ails
Need to pay attention to press on nails
o ID band/allergy
Essential. They will be facility specific.
o Jewelry - rings
It needs to come off. Especially necklaces and earrings
We eed to ake sure they do’t get a eletrial burn
Rigs do’t eed to ot eessarily oe off if it is't aroud the teperature.
You need to make sure the rings is taped on with the stones tightly together
Piercings must come out because of risk for burn
o Contacts/glasses, dentures, hearing aids
We dont want the eyes scratched
Make sure the glasses are labled if they really need them
If you take out the dentures the shape of the face changes so we want to keep them
in unless they are an obstruction to the airway
Hearing aids have to be sent with your patients. Bring with a cup so we can take them
out during surgery
o Void prior to OR
Anesthia can make people incontent
o Preop Medication- Oral vs IV/IM
