The epidemic: 37 million hiv infected people in the world, over 2/3 are in africa, 1/1 million aids death yearly, 1/20 infected in africa, 1/200 in the united states. Which structures determine who is most vulnerable: cash economy, limitations on government capacity to respond, gender relations, public responses to epidemic. Effects of cash taxation: male labor migration. Increased labor on land for women and children. Increases in stis: transition to cash economy. Structural adjustment: user fees (co-pays) for health and education, privatizing government services, currency devaluation, reducing or eliminating. Logic of austerity: brings efficiencies of the market into the government bureaucracies, directly reduce government spending, boost production by encouraging business investment, reduce in-country demand, more production, less consumption = better balance of trade. Transactional sex: having sex because you owe it. Possible hiv causes: availability of health care was limited, unsafe medical practices.
