Lily is 16 years old; she is being seen today in an outpatient clinic where she has been referred by her family doctor. The reason for the referral is her recent self mutilation behavior. Lily has cut her left wrist several times in the past, but now she is leaving significant scars and her mother is very concerned. Lily"s mother describes their relationship as difficult even when lily was a young child. She describes lily as a child who was dependant on her, and she at times resented that dependency. The relationship is filled with shame and guilt as described by both lily and her mother. She tells the nurse that cutting helps relieve the feelings of being empty and lonely. Lily also states she has never had an intimate relationship, as anyone she has liked has always failed her.
