Purpose of the study was to identify system-wide factors contributing to falls in community-dwelling octogenarians. 8 falls were investigated using the systemic falls investigative method. Across-case analyses identified 247 contributing factors grouped within 4 distinct themes: everyday living has become risky, supervision limitations, health care system disconnects, poor fall risk identification and follow-up. Most elderly people would rather live at home with increased level of support as opposed to moving into ltc: this strategy is much more cost efficient. Main reason for transition into ltc is an adverse event e. g. , a fall. Falls cost the canadian health care system billion in direct expenses in 2004. As age increases, the risk of a fall causing an injury increases: up to 9x 65+ Multifactorial fall risks: decreased vision, history of falls, gait and balance abnormalities, neurological impairments, decreased strength, poor footwear, postural hypotension, environmental hazards.
