Role of nurse in mental health and psychiatric assessment. Comprehensive assessment = collaborative activity involving all members of the interprofessional health care team. Purpose: to establish a baseline understanding of the patient"s concerns and needs, and her strengths and resources. Includes: health history and physical and mental status examination. Questions about harm to self or others. Important to ask the safety questions first, and leaving the presenting concern for last: acute situations risk for injury with psychotic states, depression, dementia, and delirium. And then perform a focused assessment later to provide the treatment team with enough info about the patient. Develop a rapport first eg. the questions i am about to ask you i ask all of my patients . Risk assessment and health promotion: health promotion should be focused on 4 important conditions that are overlooked: anxiety, depression, suicidality, and dementia, anxiety. Many people seeking primary care have mental health concerns. (eg generalized anxiety disorder (gad) is very common)
