Ch 78 drugs for peptic ulcer diseases: Peptic ulcer disease (pud): group of upper gi disorders characterized by many degrees of erosion of gut wall mostly caused by infection w/ helicobacter pylori. Severe ulcers can be complicated by hemorrhage & perforation. Develop when exposed 2 acid & pepsin; most common ulcer in lesser curvature of stomach & duodenum. Peptic ulcers develop when an imbalance between mucosal defensive factors & aggressive factors. Major defensive factors are mucus & bicarbonate. Major aggressive factors are h. pylori, nsaids, gastric acid, & pepsin. Defensive factors protect stomach/duodenum from self-digestion (intact defenses = unlikely ulcer) Compromised defenses = aggressive factors can cause injury. 2 important agents that weaken defenses: 1. Sufficient bf to gi mucosa cells to maintain mucosal integrity. Provide more protection by suppression gastric acid secretion.
