Polio epidemic in 1916, vaccine developed by jonas salk. It was unclear whether the benefits outweigh the risks: one possible design: children with parental consent would form the treatment group and get the vaccine. But higher income parents would consent bias created against vaccine, because children of high income parents are more vulnerable to polio (children who live in more hygienic surroundings do not develop antibodies to protect against severe polio) Other design: treatment and control group are both drawn from children both parents consented to the vaccination. Carefully designed chance procedure groups resemble each other very closely randomized controlled . Use of placebo: children in control given an injection of salt dissolved in water. Randomized controlled double-blind design reduced bias to a minimum. In some cases of cirrhosis of the liver, patient may start to hemorrhage and bleed to death. One treatment is to redirect the flow o blood through a portacaval shunt.
