Public knowledge, public health measures (vaccinations), poverty reduction and a small effect of medical cures reduced disease. Decrease in disease = population surge = easier for disease to spread, and diseases persisted where poverty remained. Increased human density (500 cities > 1 million) More wars over resources more disease.
