An assessment is a collection of information and provides an insight into the patient. Three types of professional judgement: diagnostic judgment: identification of an actual/potential health problem, therapeutic judgement: decisions about intervention, outcome projection and evaluation, ethical judgement: identification of an actual/potential moral problem. This involves the patient, their family or other care providers: nurse analyses assessment data in determining a diagnosis. Which is documented and used in therapeutic decisions. 3,6 or 12 months have passed since the last visit. It screens patterns and evaluates previously identified diagnosis: admission assessment: this is the most complex assessment because it is done on new patients and requires assessing for a large amount of information. Admission assessment information is used in patient care, to help make nursing staffing decisions, used in health statistics and nursing research.
