Document Summary

Diagnosis of periodontitis does not necessarily go with diagnosis of gingivitis. Diagnosis not in notes, does not exist medicolegally. Description of disease: allows pt understanding, communication with colleagues. Anatomy: free gingival margin, free gingiva, attached gingiva. Signs/symptoms: clinical, radiographic: clinical examination, visual examination: variation from healthy, plaque accumulation, periodontal probing: pocket depth, recession, bop, clinical signs/symptoms, colour, bop, plaque, probing depth/clinical attachment, mobility, furcation involve. Conditions that alter host response to plaque and therefore course/progression of disease: diabetes, smoking, puberty/menopause/pregnancy, osteoporosis. Probing depth (pd), recession (rec), clinical attachment level (cal = pd + rec), bop. *classification = tool used in epidemiology; diagnosis = what is happening to individual pt. Severity: mild (<30% bone loss), moderate (30-50% bone loss), severe (>50% bone loss) Generalised slight/moderate chronic periodontitis, worse in molar teeth. Generalised moderate/advanced chronic periodontitis, more so in maxillary area. Supragingival calculus unrelated to plaque except it is plaque-retentive (predisposing factor) Generalised moderate gingivitis with supragingival calculus particularly around mandibular anteriors.
