Tick-borne diseases
Tick bites Tick bites can cause …
Allergic reactions Local itch, swelling, pain
Urticaria
Anaphylaxis
Allergic reactions to red meat and gelatin
Meat carbohydrate alpha-galactose is
found in tick guts/ saliva, hence tick bite allergy also causes
meat and gelatin allergy
Local secondary
bacterial infection
Local granuloma Retained tick mouthparts cause this
Transmission of
tick-borne diseases
If tick becomes engorged with blood, it causes
this
Tick paralysis Rare in humans, more likely in children >
adults
Located along Eastern coast of Aus
<5 cases per yr in Aus
Only ixodes have potent pre-synaptic
neurotoxins in their saliva
Ascending paralysis (progression may take
days)
Tick removal methods
Correct Use pointy tweezers to remove ticks
Tweezer tips should be on or as close to tick
head as possible
Pull upwards with slow, steady motion
Alternatives to pointy tweezer removal
Physical - half hitch knot under tick's body,
then pull out
Thermal - liquid nitrogen or ether spray (ie
Aerostart, Wart-Off Freeze)
Chemical - permethrin cream (ie Lyclear), then
remove
Incorrect Do not use normal blunt household tweezers
Do not use twisting or jerking motion to remove tick
Do not crush, puncture or squeeze tick's body
Do not apply petroleum jelly, kerosene, petrol, nail
polish, lignocaine jelly
Do not apply heat with match or hot nail
Do not handle tick with bare hands
Management
Once a tick is removed, look for more ticks (scalp, behind ears, natal
cleft, under breasts, scrotal region)
Check tetanus status
Give tick bite site advice to patient
Use anti-septic to keep site clean
It is normal for tick bite site to remain slightly itchy and lumpy for
several weeks due to immune reaction to tick saliva
Possible foreign body reaction if retained mouthparts
Monitor for secondary bacterial infection & development of
febrile illness should warrant return for assessment and oral doxycycline
