Consists of recurring, unwanted thoughts, ideas, or sensations (obsessive) which cause the affected person anxiety or distress. Individuals may want to relieve these feelings by performing repetitive tasks (compulsions) These obsessions and compulsions are time consuming and interfere with daily activities. Stimulus (internal or external) > obsession, distress, and anxiety > temporary relief from distress and anxiety, and ritualized behavior compulsions. Involves pathophysiology of a circuit that connects parts of the prefrontal cortex to subcortical regions and regulation of sensory motor behaviours. The circuit is called cortico-straita-thalamo-corticol (cstc) circuit. When this circuit is activated, glutamatergic signals from the prefrontal cortex excite the striatum, which reduces inhibition of the thalamus because of gaba, and thee subsequent excitation of the prefrontal cortex. The anterior cingulate cortex and orbitofrontal cortex are important for goal oriented behavior. Including the prefrontal cortex there is increased activity shown in ocd patients. Caudate nucleus of the basal ganglia is responsible for controlling habits.
