Nph = regular insulin + protamine, which is a large pn that decreases solubility and slows absorption. Insulin glargine (lantus) is newer and not cloudy. It microprecipitates due to low solubility at physiologic ph. It is not given iv and cannot be mixed with other insulins. 10 15 min 60 90 min. Neuroglycopenic: shortage of glucose (glycopenia) in the brain, usually due to hypoglycemia. 5. 0-10. 0 (5. 0-8. 0 if a1c targets not being met) (2hrs after eating a meal) Side effects: weight gain, renal excretion: dose adjust, @ risk for hypoglycemia, lose effectiveness over time, primary/secondary failure: don"t respond to tx. Beta blockers, and hypoglycemics can make hypoglycemia worse. Moa: releases gi hormones that cause beta cells to insulin secretion, which uptake of glucose, postprandial glucagon ( release of glucose from liver. Moa: liver glucose production, absorption of glucose, body"s sensitivity to insulin. Side effects: gi, titrate slow to min. effects, lactic acidosis with renal dysfunction.
