A belief that your prescriptions, discharge summaries and lab results magically float through the air from doctor to doctor and facility to facility is a complete fallacy. Interoperability of healthcare information for years and it doesn"t yet exist. I believe a huge part of the problem is that the attempts so far have essentially excluded patients from the equation. There is a pretty solid belief that individuals will not engage in the process so systems need to find a way to transfer the information assuming their lack of involvement. I believe this is why attempts so far, have fallen short and we have been at a bit of a stand still for some time now. They most often offer filtered information in a clinical format and i hear regularly from patients that it is frustrating and cumbersome. For improvements to be made in the disconnected systems of corporate healthcare, patients must be involved in the process.
