Rupture of plaque or thrombotic event that leads to symptomatic ischemia or infarction. Deposit of lipids, calcium, fibrin and other substances artery. Initiate progressive inflammatory response and process: production of a fibrous atherosclerotic plaque. Plaque is fine if it sticks there, but if it"s ruptured, it can vessels and create a thrombus. (coronary stenosis) Artery of the heart or chd coronary heart disease. Strong association btwn preventable and non-preventable risk factors. Non-modifiable: previous history, family history, men<55 & women<65, age, gender, socioeconomic factors and ethnicity. Modifiable: 90% of risk to initial acute mi. Diagnosis of chest pain: assessment of pain, pt history and risk factors, ecg. Biomarkers: ck (present in heart, brain and skeletal muscle= not specific) Tropinins: only in cardiac muscle, most sensitive indicator of myocardiac damage. Ecg changes of injury/ischemia or acs sx = infarct. Unstable/ruptured plaque = abrupt closure of coronary arteries.
