Prevent damage to the smaller vessels with stronger medications. Where the catheter gains access into the body, the actual vessel is accessed directly under the skin. Catheter does not gain access into the vessel directly from the original access point, instead it is threaded from a lower position, underneath the skin to the svc (for example) Puncture site, skin above clavicle (entry site), is dilated. Distance to right atrium is measured & appropriate length catheter is selected. Tip of central line threaded into large vein into the heart. Other end of peel-away sheath is tunneled under the skin to reach the exit site. Other end of catheter is moved through the sheath, then sheath is removed. Cvc needs to be kept in place for long periods of time - best interest of patient safety that we make sure we min the chances of the devices to be removed/dislodged.
