What is a lhin local health integration network. 14 not-for-profit corporations work with local health providers/community members to determine health service priorities for our region. Vision: take care of people in the community getting them out of the hospital and providing care within communities. Province released 2 million $ - spread throughout province isn"t that much. May 13, 2015, ontario announced a 3-year plan to improve and expand home and community care. Roadmap goals: put patients and caregivers first, improve patient/caregiver experience, drive greater quality, consistency and transparency, plan for expand capacity, modernize delivery. Patient focus hospitals filled with bugs and diseases. Mental health and addictions system wide focus. System capacity and seamless transition to avoid alternate level of care.
