Learn as linear- but in reality it is continuous and we move back and forth between steps: assessment. The gesturing and analysis of information about the client"s health status. Collection and verification of data from a primary source. Interview: orientation phase, working phase, termination phase. Concept mapping: clients with more than 1 health problem- the majority of what you will encounter, visual representation that allows you to show connections between a client"s many health problems, nursing diagnosis. Determines health problems within the domain of nursing- not a medical diagnosis. Example: a patient/ client with copd- possible to have nursing diagnosis of ineffective airway clearance . Provide the basis for selection of nursing interventions to achieve outcomes for which the nurse is accountable. Based on professional standards, ethics, and laws.
