Chapter 6 social work and the health of Canadians
Universal public health care: is a system all Canadians know and benefit from today. arguably
the best health care system in the world. Prior to the late 1940’s, before the fight for universal
health care began in earners, access to health care was based solely on one’s ability to pay
-provincial. governments were slow to move towards universal health care and only reacted
under pressure from non-government groups.
marsh report: 1943, detailed the need for comprehensive universal social program including
health care. escribed as most import document in the history of the welfare state in Canada
Pressure to implement universal health care
-British North American act of 1867 established the jurisdictions of the federal and provincial
governments in delivering health care services.
-from 1880-1950’s there were a variety of pre-payment health plans across Canada, sponsored by
local government, industries and volunteer agencies
-by 1934 there were 27 hospital sponsored pre-payment plans in 6 provinces
-1939 the first Canadian blue cross plan was formed in Manitoba
-1947-first public insurance plan for hospital services was instituted in Saskatchewan
-1961 all provinces and territories has signed agreements with the federal government for
limited, in-patient care with federal cost sharing
in 1962, Saskatchewan was the center of hard fought struggle between the provincial government
under the leadership of tommy Douglas and the medical establishment.
Hall report: disclosed that 7.5 million Canadians did not have medical coverage and
recommended a comprehensive health service patterned on the Saskatchewan model,
Medical care act (1968), by 1972 all provinces and territories has extended their plans to include
physician’s services, although physicians were permitted to opt out” and extra billing was
permitted.
Canadian health act: (1984) changed the funding structure, prohibited opting out and extra
billing, and further strengthened the universal nature of the public health care system in Canada
Romanow commission, the final report was released in 2002, his work said he was guided by
two things, Canadian values of fairness, equity and solidarity and by the evidence.
- This approach led him to recommend expanding public, not for profit Medicare to
include home care and pharma care. And to introduce a genuine system of primary health
care
- His recommendations were premised on three main theme 1) strong leadership is needed
to maintain Medicare 2) the system should become more efficient and responsive and 3)
both short and long term-strategies are need to maintain universal healthcare.
Medicare: each province must meet the following 5 principles to get funding
- 5 principles of Medicare, public administration: the health care insurance plan in each province
much be administered and operated on a non-profit basis by a public authority responsible.
comprehensive: the plan must cover all insured health services provided by hospitals, Medicare
practitioners or dentist, universality: 100 percent of the insured persons of a province be entitled
to the insured health services provided for by the plan on uniform terms and conditions.
Portability: residents moving to another province must continue to be covered for insured health
services by the home province during any minimum waiting period imposed by the new province
and accessibility: the health care insurance plan of a province must provide an insured health
services on uniform terms and conditions and reasonable access, reasonable compensation to
physicians and dentist or all insurance health services rendered.
