Senate meeting, he changed clothes and shoes (because senators wore a special kind of mantle and tunique, and wore red shoes). Then went out with wife and they were ambushed. Milo had to defend himself, and while defending himself, killed a man. What cicero is stressing is the fact that the other one was there without his wife, his usual companions, etc. The man clearly meant to ambush him since he had no loved ones with him, while milo had his wife with him.
