PHAR 100 Lecture Notes - Lecture 4: Calcium Channel Blocker, Angina Pectoris, Nitric Oxide Synthase
Document Summary
William heberden (1768): english physician who described angina pectoris as, those who had a pain in the chest when they walked. Lauder brunton (1857): amyl nitrate introduced, administered by inhalation and relieved anginal pain for 30-60 seconds with the action only lasting 3-5 minutes and was difficult to adjust the dosage. William murrel (1879): sublingual nitroglycerin (glyceryl trinitrate) introduced with vast improvement as it lasted 20-30 minutes. More drugs found in the last 50 years. Due to lipid deposits in coronary arteries, causing blood flow to decrease (atherosclerosis) Sudden, severe pain due to less blood flow and thus less oxygen supplied to the heart muscle. In order to decrease pain, you either decrease oxygen needed by the heart or increase the oxygen being supplied. Precipitated by 4 e"s which increase oxygen demand: eating, exercise, excitement, exposure to cold. Oxygen requirement increases heart rate, force of contraction, arterial pressure and volume of blood to be pumped which occurs during exercise.
