DAY 10
July 20th, 2017
North American (western hemisphere)
- Breast fed babies are smaller than bottle fed babies
- Since bottle fed babies can have milk as much as they want and when every they want
Growth and Nutrition: Infant nutrition
- Prenatal experience and early infancy nutrition and crucial for healthy growth
- Breast-fed infants grow differently than formula fed infants
- Supplements given between 4 and 6 months, but wide variation across populations
- Weaning periods is that most vulnerable for growth and development
Stress During Labor article
- Stress surviving poor conditions/situation
- Lactation is not essential for survival of MOTHER
The difference between malnutrition and undernutrition
Malnutrition: bad nutrition both over and undernutrition
Undernutrition: insufficient intake of nutrition
o Associated with growth faltering or retardation
- Back to the Toba why do their growth trajectory fall off after the first year?
- Malnutrition begins during weaning in most developing countries, and continues
throughout childhood
Consequences of Inadequate Nutrition
Protein-energy malnutrition (PEM)
- Affects 50% of children worldwide
- Marasmus early < 18 months of age
o severe PEM and caloric deficiencies
o inadequate nutrition, infection, dehydrations, circulation disorders
o severe energy deficiency
o characterized by think limbs, very emaciated, very low weight for height (more
than 350 from population mean)
o if untreated, can be lethal
- Kwashiokor developes after 18 months as a mother weans child into a diet low in
protein, rich ___ and carbohydrates (high corn diet)
o characterized by edema (fluid build up), think limbs, enlarged belly, impaired
overall health and mental well being

- Energy “efficient metabolism” in the world of McDonalds and the internet
- A major problem in affluent countries, but also in less privileged populations
- Result of low levels of physical activity coupled with a hypercaloric diet
Growth and Socioeconomic Status (SES)
- SES income, occupation, education, housing, household goods
- Nutritional status
- Disease load access to medical care
- Psychosocial stress
Psychosocial short stature (psychosocial dwarfism, or Kaspar-Hauser syndrome)
- Genie Wiley, the feral child
- Locked in closest for 10 years
- Cried a lot as a baby so her dad locked her in a closet
- He didn’t show her any affection/love/nurturing
- After 20 months, just left her there and tied her to a totty chair

- Dad would forget to feed her or take her out of the totty chair
- Stop it AND no more only words she knew
- Was found at 13
- 5’4 and 65 pounds
- children restricted environments, can catch up
Childhood Growth and Development
Learning Objectives
- to understand what growth canalization is and the difference between early and late
developers
- to define catch up growth and understand when it can occur
- to discuss sex differences in growth, and the speed of growth during adolescence
- to describe the major components of HPG axis, and how the HPG axis regulates
reproduction
- to describe the role of the HPG axis in puberty
Nobody is free from environmental constraints:
Illness: appetite loss, weight loss
Lower centile in weight
- Yet risks are not evenly distributed within and between populations
Growth Canalization
- A tendency to keep to a narrow and predictable track of growth, usually along a centile
line
