NURS 233 Lecture Notes - Lecture 5: Preterm Birth, Blood Transfusion, Fetal Position
233 perinatal Week 5 February 1st
give narcan if the baby was given fentanyl so if they were over sedated. Ex. if you were
given morphine and then the baby came faster than expected then the baby is sedated
then you give narcan.
But if the mom was taking narcotics ex. cocaine then you don’t give the baby narcan you
want to give them narcotics and wean them off of it.
Fetal fibernecton is a good and easy test but looks really cool.
Know the difference between preterm labor and miscarriage because you need to know if
they are in labor or if the mom and baby is at risk.
If the person says they have active chlamydia or syphilis then they will most likely do a C
section to protect the mom and baby
Know hemoglobin and how that changes in pregnancy. Significant
decrease in hemoglobin at the end of the 2nd trimester (97-148 g/ml) is normal - Non-
pregnant values are 120-158 g/ml
Know vaccinations and tests
Case 4 is G3T1P0A0L1
)f normally don’t give a blood transfusion if the hemoglobin is above 80 but if its
below they will diffuse blood after delivery.
Why isn’t MMR safe in pregnancy? Because it’s a live vaccine. Only not live vaccines
can be given in pregnancy
Midterm is up to pregnancy
F)VE P’s
Factors that affect the process of labor and birth:
• Passenger (fetus and placenta)
• Passageway (birth canal)
• Powers (contractions)
• Position of mother (is she laying or walking or kneeling etc.)
• Psychological response
• Also consider the availability of labor support and where they are delivering
and the care personal.
Passenger:
• Size of fetal head
• Fetal presentation (head first, bum first)
• fetal lie (relation of spine of baby to the spin of the mother (either cephalic or
breach, transverse, horizontal)
• Fetal attitude (relation of the body parts to one another, the fetus body parts,
ex. if they are curled up. You want them to have chin tucked and in a flexed
position)
• Fetal position
