Observe for expected signs of aging- wrinkled skin, thin grey hair, aging spots etc. Watching their gait and posture, mobility gait: tends to have a wider base, steps are shorter and uneven. Posture: general flexion observe for nonverbal cues. Watching their facial expressions for grimace and pain clutching a body part. Observe their breathing effort (shortness of breath = listening and looking). Listening to their tone of voice and mood, for adventitious breath sounds (stridor). Assess skin colour and level of consciousness, nutritional status and personal hygiene. Observe for any deformities or unexpected physical findings. Perform a safety check before beginning your health assessment (assess behaviour) Temperature: expected range: 36 degree celsius - 36. 8 degree celsius (oral). Heart rate: expected range: 60-100 bpm; but lower than that of the average of the younger adult - Assessed by the apical pulse for 1 full minute (only if irregular, otherwise 30 seconds of radial pulse x 2 is okay).
