Begins with the initial patient encounter and develops throughout the duration of care. The primary source of data collection is the patient, but some situations may cause you to rely on other sources: e. g. Secondary sources such as family members, friends, neighbors, police, health care workers, and medical records. When getting data from kids, get it from a variety of sources. Developmental stages and levels should be considered (mental health disorders often cause children to regress in their developmental stages) Assessment of children should be a combination of interview and observation: headass psychosocial interview technique: Consider the importance of confidentiality- they may not want their parents to know. But: threats of suicide, homicide, sexual abuse or behaviors that put others at risk must be shared with other professionals and the parents. Know that as we age, the five senses start to deteriorate.
