What is a medical history: structured/systematic evaluation of the patient to gain insight into the patient"s health status. Includes: current health issues, past health issues, general health status, other factors (health, socioeconomic, lifestyle) that may impact their current condition and/or response to treatment. Important for how much they can afford but also what their support network is like: family history, objective: determine the problem and develop, patient perspective/goals. Ice: ideas, concerns, expectations: primary diagnosis, differential diagnosis. Comprehensive history: chief complaint, systems review, personal history, family history. Daily, weekly, monthly: not just pain but also suffering - could have something else going on that intensifies the pain. Systems review: filled out in the waiting room before they see you, purpose: thorough head to toe examination of patients health status. Personal history: goal: gain a more through knowledge of the patient"s previous medical history and lifestyle.
