- gastric erosion and ulceration
o erosion: loss of gastric mucosa b/c death of epithelium
o ulcer: deep erosion that penetrates the muscularis mucosa
- peptic juices in stomach: HCL and pepsin secreted by gastric glands
(foveolae)
o protein digestion and kills unwanted microbes
o mucus secreted by outer lining and protects from autodigestion
- most nutrient absorption in SI and fluid absorption in LI
Signs of Gastric abnormality
- pain (dyspepsia)
- heartburn (acid reflux, gastroesophageal reflux GERD)
- vomiting (emesis hence antiemimetic)
- Melena: latin = melon (means black, dark)
o Means dark tarry feces
- Ulcer: cell death of the lining of epithelial structure
o Gastric ulcers: gastric mucosa = folds that helps stomach expand
o Ulceration imbalance between mucosal defence and aggressive
forces
Normal forces: acid and pepsin
Stomach ulcers
- gastric ulcer stomach, leads to perforation, haemorrhage, obstruction and
pain
- Duodenal ulcer duodenum (most common)
- Peptic ulcer peptic juice
- Complications
o Pain
o Perforation
o Haemorrhage
o Obstruction
Normal stomach (refer to diagram)
- lots of glands in the surface of stomach
- little columns of fluid (acids) squirted into the lumen
