Pain and bloaing: diarrhea and consipaion, could be alternaing with each other. Cause: can be triggered by stress/anxiety, more common in women. Do not give anidiarrheals for long term use. Oten present with abdominal pain not yet diagnosed. Perforaion may occur in less than 6hr from onset (interrupion of intesinal blood supply obstrucion/arterial embolus) Diferenial hypotheses rule in/rule out: use the oldcart pneumonic, onset of symptoms, locaion, duraion, characterisics, aggravaing factors, relieving factors, treatment before arrival. Elemental diet (high calorie and protein, fat free, no residue/iber: increase gradually to low residue/iber, low fat. Sulfasalazine: both, decreases diarrhea, pain and rectal bleeding. Remicade can reduce number of istulas in crohn"s: prevents joint damage, remission. Re atach small intesine weeks later, gives it ime to heal. 2 stage surgery: remove diseased bowel, temporary ileostomy: surgical closure later. Fairly normal bm 3-4 imes per day ater surgery.
