Early medical records: simon forman and richard napier applied astrology to medicine around, kept detailed medical records for astrological calculations, although they practiced dangerous medicine, ~50,000 records are available in digital form today. Important source of data in medical practice. Paper-based medical record (pmr): still common, especially in primary care. Advantages of pmr: portable and intuitive access, little structuring is required for data entry. Can add an extra piece of paper with notes, or make notes in the margins etc: minimal training required. Everyone is familiar with pen and paper. Disadvantages of pmr: only one person can use a particular record at a time, difficult to read other peoples hand writing. E. g. , finding all patients with the same diagnosis. Can"t support anything more than plain record keeping. Pneumatic tube system: was used at mayo clinic, one central storage for the entire campus, fast record retrieval, expensive, no off-campus access. If the physician lost the record very big consequences!
