Was the rst longitudinal examination of death rates. Causes of death have changed, called an epidemiological transition***. This is the transition from one type of disease death to another (ie. the movement from infectious to chronic disease. Clear and growing gradients of health identi ed, despite development of national health care programs and welfare systems. There are differences and divisions between the health of the rich and poor. There is a growing health gap, which remains stagnant. Natural or social selection (reverse causality, where the poor get sick, the rich get healthier). Behavioural (agency) factors, where poor people engage in worse behaviours (ie. smoking) resulting in poor health outcomes. Compositional: aggregated characteristics of individuals in an area (ie. relationship between income and health, bene ts of good income).
