Chronic illness: health problems that persist over extended periods of time and usually (but not always) associated with disability. Determinants of health: interactions between social and economic factors, the physical environment and individual behaviour which influence health. Income and social status: social support networks, education and literacy, employment and working conditions, social environments, physical environments, personal health practices and coping skills, healthy child development, biologu and genetic endowment, health services, gender, culture. Disability: difficulty in functioning at the body, personal or societal levels. Health: a state of complete physical, mental and social well-being, and not merely the absence of disease or infirmity. Disease: condition that a practitioner views from a pathophysiological model. Multi-morbidity/comorbidity: several medical conditions in the same person at the same time. Shared decision making: a decision making process jointly shared by patients and their health care provider, in which the provider is the clinical expert and patients are experts regarding their own lives, values and circumstances.
