The type of alloreaction in transplantation depends on the type of tissue transplanted. Blood transfusion is the most common transplant in clinical medicine. Crossmatch testing to examine compatibility between recipient and donor. Anti-hla antibodies arise from pregnancy, blood transfusion, or previous transplants. Chronic rejection of organ transplants is due to the indirect pathway of allorecognition. Direct vs. indirect allorecognition: t cells do not directly recognize transplanted cells but subcellular material that is processed and presented by self-mhc molecules. Indirect pathway of allorecognition leads to production of anti-hla antibodies. Improving the outcome of organ transplantation: select donor who is as similar as possible to the recipient in hla class i and ii. Hla matching also ensure effective reconstitution of the adaptive immune system: patient receives bone marrow transplant of completely different hla than own.
