If counsel can"t get instruction from client, then deem unfit. Even if mental disorder specialist deem unfit, lawyer get final say: understand nature & object of the proceedings, understand possible consequences of proceedings, communicate with counsel. But have to be expert to come up with diagnosis. For fitness it doesn"t matter what the diagnosis it, as long a mental disorder exist. Hard to do research on validity of informal interviews. In a lot of cases, it"s a matter of fit or unfit. Need to be expert if it"s in-between.
