Compliance: extent to which a person"s behavior coincides with medication or health advice, correct dose, time, avoiding prohibited substances, poor compliance: dose omission/supplementation, wrong time, stopping/not starting therapy. Consequences: worsening symptoms, develop complications, premature disability. Poor compliance: common with chronic therapy, preventative treatment, rates are relatively higher t acute conditions, short term therapies. Patient related factors: demographic not predictive of compliance, age not significant, but associated with polypharmacy, physical/cognitive limitations, access to medications, co-morbidities, predictive factors, psychological problems, cognitive impairment, social isolation, perception of illness, treatment efficacy, adverse effects. Treatment related factors: complexity of dosing regimen, better: 1 or 2 times per day, avoid interruption of normal routines, number of drugs (polypharmacology) inconvenience. Combination pills lose flexibility of different doses: side effects, container packaging/design, palatability. Improved compliance: simplified dosing schedule, extended half life, ease f contact with physician, continuous supervision, patient education on seriousness of disease/efficacy of medication, drug has minimal adverse effects, written instructions.
