Web Form Creation 4/24/2013 11:53:00 AM Lecture 5 Web Forms  Online forms offer: o Speed – quicker deployment, wider audience o Efficiency – collection of data o Anonymity – leads to frankness o Automation – format diverseness of data o Cost-effective process – no paper or postage o E-commerce add-ins  What makes up a form: o Form:  Adding questions  Layout, form elements o Email template:  Submitter  Form owner o Processing:  Action – what should it do when submit is clicked o Data collection:  Store your results  What format Building Forms  Online form builder – an application that builds your form based on your requirements, form is hosted on their server o Drag and drop features o Customizable form templates/themes o Send replies to form submitter and form owner o Capture information in a database (.txt, Excel, MS- Access) o Data is stored on their cloud based service – stored online and can be accessed from anywhere, anytime o 128 bit SSL encryption o Integrate payment collection with Paypal, and other ecommerce sites o Can have high costs o E.g. – WuFoo, Zoho Creator, Jot Form  Open source form builder – provides you with the application to install on your computer/web server to create your own form o Don’t host any part of your form or data o You have full control and privacy over your data o Technical background is needed – need to be able to understand the server and installation o E.g. – Form Tools, Mach Form  Build a form yourself: o A web provider hosts the web pages and provides instructions on how the form will be processed o You are responsible for creating:  A form with web editing software (Dreamweaver, HTML code)  An email template – what replies go to the submitter and form owner  Collection database  Thank you webpage  Access to server’s cgi scripts to process the form (web host provides this)  Reporting is done when data results are imported 7 Steps to Creating Your Own Form  Create the form o Extension .html o Use web authoring or HTML o Set up form (red outline in Dreamweaver) o Add questions and form elements:  Submit button – mandatory  Text field – single line (name)  Text area – enter multi-line comments  Checkbox – check more than 1 item  Radio Button – check only 1 item  List/Menu – drop down selection list  Button Name – used to submit or reset  A Form element is identified by:  ID – assigns a name to the form element  Database field name and used in the email template  Won’t show up in the document window, will in property panel and email template  Label – appears beside the form element (text box, radio button etc.)  Appears on the form itself  Attributes  Box width, number of characters accept  Changed through Property panel  Create the e-mail template o Used to send you the form results o Requires the ID names used in the f
