A failure is always an engineering lesson hopefully it doesn"t cost anyone their lives: multiple incidents of engineering failure with casualties. Challenger disaster: exploded 73 seconds after launch caused by a srb colliding with the central fuel tank, the crew capsule detached and plummeted to the ground intact. Increased attention from the media because there was a civilian on board. Reagan in 1984 putting a school teacher in space: christa mcauliffe. Rubber was supposed to withstand the pressure and contain the fuel inside. Rubber not at optimum temperature disastrous consequences. The resulting stress on the side of the rocket caused a hole, and fuel began pouring out: investigation. Led to a number of changes at nasa. Findings rushed the launch, rings could have been improved or fixed or avoided, etc: rushed launch media attention from mcauliffe, etc. A culture of worrying to cause delays was identified by a presidential committee.
