- What?
oWritten guideline for conduct of care
oTool nurses use to record actions that will be used with client
- Why?
oMust communicate amongst nurses and other members of HCT
oCommunicates info about goals and assessment data and expected data
oIdentifies interventions and rationales
oEvaluates the plan
oActs as a blueprint for nursing actions
oGives direction for implementation of care
- When?
oDeveloped after comprehensive patient profile has been established
oEx. reviewing problem and history of client
oMay include physical assessment, lab test results, etc.
