Was four steps, but now five steps. Knowledge to apply to the unique individual. What is contributing to it: write a statement. Plan nursing actions: understanding what the clients present health status is. Prioritize what is going on with the patient: this will help us plan our nursing actions. Planning outcomes - what is the patient"s goal: work with the client to choose the desired outcomes, need to remember values of self determination.
