Discharge planning and care transition from hospital to home. Outline: discussion of issues r/t discharge/transitions, review and apply health promotion lens to discharge: hospital to community, transfer of care note assignment. Reality: when should discharge planning begin, when the patient is admitted, in the ed, ask- what is the plan of care. Include: the patient and family as full partners in the discharge planning process, why, family acts as main support. 2: family can be care provider, might be poa, help with lifestyle changes, see who the patient views as family, family dynamics. Listen concerns: eye contact, face to face, reiterate what they said. At every stage in life, health is determined by complex interactions between social and economic factors, the physical environment and behavior. They do not exist in isolation from each other. It is the combined influence of the determinants of health that determines health status (public health agency of canada, 2011)
