Accounts for: 2-5% jaundice, 15-30% of fulminant liver failure, 40% of acute hepatitis, >50 yrs age. Backflow into hepatic portal vein & artery & high bp here. Small bvs around the gi tract (i. e. esophageal bv) will rupture. Blood can"t circulate through liver and leave via hepatic veins. Endoscopy will tell you if there"s portal htn: no need to measure if: stool has occult blood (from ruptured bvs) or person has history of alcoholism (can anticipate) Functions of normal liver: synthesis of liver proteins, regulation of blood glucose, bile formation and excretion, detoxification of xenobiotics and endogenous compounds (main site) Liver pathology: hepatocyte damage signals by the rise of alt, disorders of biliary system bilirubin and alp rise. Ast also rises but not specific bc also produced in heart. Mostly in small intestine but stomach can metabolize alcohol to some extent.
