Document Summary

Determined by severity of pain to be managed or range of pain (anatomically) Mild to severe pain (short duration in a specific/small region of the body) Very acidic (hard on stomach, gi tract) Prostaglandin are proinflammatory molecules made and broken down locally in virtually all tissue in the body. Pgg2 are synthesized by cyclooxygenase (cox-1 and cox-2) from arachidonic acid. May be more selective for cox 2. Has an active metabolite which may reduce the expression of inflammatory mediators il-1 and il-6. Mechanism of action is not fully appreciated. This might be the more important mechanism of action, rather than the inhibition of cox-1/cox-2. Has little activity on peripheral cox-1 but works in the cns. However, it has little gi side effects. However, overdosing can cause severe life threatening toxicity. Acetaminophen is broken down and conjugated to glutathione in the liver. However, glutathione in the liver can be used up. Then it go into another pathway and make free radicals.
